34 year-old female who had undergone (420cc) saline implantation via the nipple areas, with implants placed subglandularly (on top of muscle) approximately 12 years earlier (also had delivered 2 children and had undergone a 40lb weight loss). She wished to be upsized with gel implants, and I recommended a pocket-exchange for under the pectoralis muscles for a "softer look", with capsule removal as necessary. Although a bit risky due to the nature of her previous surgery, I also recommended a proper mastopexy at the same time. She was upsized to Sientra smooth, round silicone 505cc implant.
